Consent Form
All schools require written consent from a parent/parents for other parties to obtain any information about your child, verbal or written. I have provided a link to a printable version of my consent form. Once completed please either fax it to me at 978-462-2922, mail it to CHC, 257 Low Street/Upper Level, Newburyport, MA 01950 or you may also drop it directly at your child’s school. If you are unable to print a copy please phone or email me and I will gladly mail you one.
